Terms of service

Last updated: 6 November 2026

These terms apply when you install and use Codform, a Shopify app run by Qays Kadhim, a licensed freelancer in Dubai, United Arab Emirates (“we”). By installing the app you agree to them.

The service

Codform adds a cash-on-delivery order form to your Shopify store and creates orders in your store from it. We work to keep it running and correct, but we provide it “as is”, without a guarantee that it will always be available or free of errors.

Plans and billing

The Free plan covers up to 60 orders a month created by the form. The Pro plan has no monthly limit. Shopify bills plans on your Shopify invoice, and Shopify's billing terms apply to charges, refunds and cancellations. You can change or cancel your plan at any time on Shopify's plan page. If we change prices, we'll tell you at least 30 days before the change applies to you.

Your responsibilities

Customer data

Orders created through the form belong to your store. We handle shopper data only to provide the service, as described in our privacy policy.

Data processing

Liability

To the extent the law allows, we aren't liable for indirect or consequential losses, such as lost sales or profits, and our total liability for any claim is limited to the amount you paid us for the app in the 12 months before the claim.

Ending the service

You can stop using Codform at any time by uninstalling it. We may suspend or end access if these terms are broken. If we stop offering Codform, we'll give at least 30 days' notice.

Changes to these terms

We may update these terms. If a change matters, we'll tell you in the app or by email before it applies. Using the app after that means you accept the new terms.

Law

These terms are governed by the laws of the United Arab Emirates (Emirate of Dubai).
